Urban Medical Group encourages patients to communicate electronically with staff and their medical providers over the patient portal for patient related health information, which can be accessed onwww.urbanmedicalgroup.com. 


Please download the free HEALOW app found on the patient portal website, which allows for quick and easy communication with your provider, and the viewing of recent labs and your healthcare summary without the need to resubmit your username and password.
 
To receive a username and password for the patient portal please call the office at 646-355-3711.
 
Any patient specific emails that are sent to this address without prior authorization from the Urban Medical Group staff will be deleted to maintain patient privacy.
 
To receive a secure patient related file from our office, please either call the office or send a secure message on the patient portal with the details.
 
To send a secure patient related file to our office, please send the documents to our secure fax server at 212-300-4989 or contact the office through the portal or phone and we will send you a link to send the file.
